🚨 MAIL-IN REQUIREMENTS

4. Authorization Form: Download, sign, and include our [Service Authorization Form] in the package.

⚠️ NOTE: If you possess a later "FBS 4" type ignition (common in some 2014+ models), this service is not compatible. Please check the label on the back of your EIS for the "FBS 4" designation before ordering.
